Pulling out

I have a problem with my swing, beginning with my inability to keep my front foot in the batters box. I am a lefty, with an incredibly fast swing, yet I cant hit anything because my front foot pulls out of the box and then my head goes with it.
How do I stop my front foot from pulling out?
What could be causing this foot to come out of the box? Could it be an inmate reaction to the fear of being hit?

Please help, any suggestions are welcome.
