More than anything this may be a result of trying to pull the ball too much. Practice keeping your head down (at least at the level of the pitch) when you swing. Don't look at the flight of the ball until you have finished following through with your swing. So basically your head should be in the same position when you were looking at the location of the pitch until the end of your follow through.
