Jimmy,

That is hard to say with out seeing his swing. But I feel most of it is the location of the ball and or timming. If he is early on outside pitchs he may reach and stay out over the front foot to make contact. The same could happen if his timming is off.

I would take a step back and hit off the tee and the bag. Check is swing at contact using the heavy bag. Then you can see on the outside pitch if his hands are leading the bat, head out, etc. Then to soft toss / cage.
