mechanical problem

Hello,

I've been experiencing a persistent hitting problem over the last several years: I am consistently hitting the ball on the middle of the bat (i.e., lower than the sweet spot). This goes not only for balls hit to the opposite field, but also some that are pulled. Without observing my swing which would be ideal, does anyone have any idea as to what the problem could be, and most of all, how I can correct it.

Is it a problem with my depth perception (vision), or could I be stepping to the plate as opposed to towards the pitcher?
