Too much uppercut

My whole team says that i have way to much of an uppercut, and for the last 2 years i have not been able to fix it. My coach says that im pullling my head off the ball and that when i do this my shoulders and everything come way up. I am known as the 'golfer on my team.' What i do know is that when i swing i arch my back like a golfers finish which causes me to swing way upward but i have no idea why. Can someone help me to understand how to match the plane of my swing to the plane of my pitch? Also when i do make contact, the ball goes way up and its a fly out to the outfielders, i am very frusterated because i know if its a liner its gone over the fence. Please help!
