I think it's question of how far outside?

I think GRC, you are referring to outside-outside pitches (on the black or further) that have to be hit "and hit often" in ML. This is part of protecting the plate. Purely reaction, and the bat has to come far away from the body. Often, the ball is hit with both arms extended and there really is a lack of rotation.

Just as often, there are pitches from middle-out that can be hit with the type of rotation Jack and others describe. Rotational mechanics with a wider arc.
