In regards to your second question, I should point out that there is a component of lead-shoulder rotation that I find overlooked by most batting authorities. I refer to this component as the "Inward Shrug" and additional rotation of the lead-shoulder associated with it. As I explain in the link below, making efficient use of that added rotation is a major difference between the average and elite hitter.
