See Dusty Baker's book "You can Teach Hitting". Most of your questions should be solved as a result. Also practice the pushing your bottom hand back and your front hand forward during your pre laungh position movement (Jack's torque drill). This creates a pulling the bow effect as described earlier on this site. This inturn causes a natural extension which ends up with a Ted Williams type finish in which the bat comes all the way through and ends up almost hitting the players back. (The fence drill also helps.)
