I haven't seen your swing, but i'm guessing that one of your problems might be lunging and a lot of movement back and forth in your swing. There isn't much you can do other than get more reps. But my biggest suggestion is make sure ur hitting from your backside, and get your front foot down before the pitch is thrown. When your front foot is down and your hitting from your backside it makes it easier to watch and recognize the pitch, because all you have to do after recognition is turn your hips and swing.
