not hitting sweet spot

I'm wondering if one of the hitting experts on this board can help me to identify a persistent problem that I am experiencing.

Everytime that I pull a ball (or try to pull a ball), I end up making contact on the thin (above the handle, below the sweet spot) part of the bat; the result is a meek groundball/popup to either 3B or SS. The only time that I've managed to make solid contact has been to CF and RF.

One thing that I've recently noticed: I have been gripping the bat wrong. My hands have been wrapped around the bat in a way that the wrong sets of knuckles are aligned. Apparently, I am "choking" the bat (gripping too tightly).

Any suggestions would be greatly appreciated.
