Proper Angle of Attack to the ball

What are some drills to teach the following:

1) The proper position of the top hand at the contact point? (Position 'A' to 'C')

2) Rhythm Hitting. Not being "dead handed" Proper loading and staying back.

3) Based book on approaches to hitting.
